Liposuction is one of the most popular cosmetic procedures, but is rarely carried out on the lower legs. Now, however, you can have France's hottest export: "cankle" removal. The majority of women have narrowing where the lower calf and ankle meet, giving a shapely contour. Some less fortunate females have little or no discernible difference in shape in their lower legs, giving rise to the term "cankles" (a melding of calves and ankles). This characteristic has nothing to do with age, height or weight. "Cankles" are inherited, just as eye or hair colour are inherited. Some women hate their "cankles" so much that they will always wear trousers and never a skirt or dress and often cannot wear zip-up boots or skinny jeans. They may try exercise, massage, diuretics for water retention, but nothing helps to reduce "cankles".

In the United Kingdom, some cosmetic clinics are offering laser assisted liposuction for women desperate to slim down their "cankles", to make them look more defined. Laser assisted liposuction involves a fibre-optic laser being inserted through a small incision in the skin. Light from the laser is then absorbed by the fat cells, which are then liquified until they are broken down into the body and flushed out naturally. The operation is carried out under local anaesthesia.

Conventional liposuction is carried out via a small incision in the skin and a cannula (a small hollow tube), through which fat is suctioned and this procedure can be used on various parts of the body. But surgeons are generally reluctant to operate on the calves and ankles, as results can be poor and it may leave an uneven, lumpy look. However, in France the procedure has been shown to be successful without the use of lasers. The technique is known as "leg reshaping by circular liposuction" and has been carried out in France for the last twenty years.

Laser-assisted liposuction is a fairly new technique and some surgeons say that it has not yet proved superior to conventional liposuction. Most surgeons will not perform liposuction on the lower legs because of the potential for complications to arise and many will not touch the "cankle" area with conventional liposuction. This is because swelling can occur and it is an area that contains superficial nerves, so the procedure can give rise to irritation and pain.

Leg reshaping by circular liposuction involves firstly preparing the area to be treated by injecting a solution containing saline and adrenaline to reduce bleeding and bruising. Several incisions are then made in the skin, through which very fine cannulas are inserted to suction the unwanted fat. The circular part of the procedure refers to the fact that this technique is undertaken around the entire lower leg superficially. Then using a back and forth motion with the cannulas, but without suction, the fat is broken up into a liquid. A small amount of fat has to be left, to ensure that the skin shrinks back evenly. After the procedure has been carried out, the patient's legs are massaged to ensure an even distribution of the remaining fat, giving a smooth finish with no irregularities. The average amount of fat removed from both legs is around 3.3 pints and the procedure costs between around 3,500 and 5,250, which includes an overnight stay following the operation.
